IMPORTANT

Due to limited food vendor space and our goal of providing a variety of food options for attendees, The Chamber St. Matthews reserves the right to limit or restrict menu items to avoid duplication and ensure all participating vendors have a successful event.

Applications WILL NOT be accepted without all requested information, payment, Certificate of Insurance, and menu details.

Vendor Space Information
Standard Food Vendor Space: 10' x 10' Vendor Fee: $300.00 All vendor spaces are located on grass. Larger spaces may be available. Contact The Chamber St. Matthews regarding availability and pricing. The vendor space only is provided. Vendors must provide their own: Tent/Covering Tables Chairs Displays Extension cords (if applicable) Any additional equipment required All food preparation and serving areas must be covered.
Food Vendor Requirements
Certificate of Insurance All food vendors are required to submit a current Certificate of Insurance with their application. Applications will not be processed without proof of insurance. Permits Food vendors are responsible for obtaining all required Louisville Metro Health Department permits and approvals and complying with all applicable health and fire regulations. Setup Requirements Food vendors must arrive no later than 9:00 AM on October 17. Vendors must be fully setup and inspection ready prior to event opening. Vendors are responsible for protecting food, equipment, and supplies from weather conditions. Utilities No electricity or water will be available on-site. Vendors must make independent arrangements if needed.
Release & Indemnification Agreement Vendor/Exhibitor agrees to hold harmless The Chamber St. Matthews, its officers, directors, employees, volunteers, agents, sponsors, and representatives from any liability, loss, injury, claim, damage, costs, or expenses arising from participation in Halloween at Brown Park.
Vendor Agreement By signing below, I certify that: I have read and understand the Food Vendor Application, Vendor Guidelines, and Release & Indemnification Agreement. I agree to comply with all event rules and regulations. I understand The Chamber St. Matthews may limit menu items to avoid duplication. I understand a Certificate of Insurance is required. I understand no electricity or water is available on-site. I understand booth assignments are at the sole discretion of The Chamber St. Matthews. I understand applications are accepted on a first-come, first-served basis and are only complete when all forms, insurance documents, menu information, and payment have been received.
